1. This store credit Award shall be used by you solely for Wellness Incentive Program award purposes. This store credit Award is only valid for the purchase of certain qualifying health and medical supplies and products on the WellDeservedHealth.com, FSAstore.com and HSAstore.com websites operated by FSA Store, Inc. DBA Health-E Commerce. This Award has no other value and may not be traded in for cash, or used, exchanged, traded or given away to any other third party in exchange for store credits, or other types of awards offered by any other third party.
2. Beginning on the month that is thirteen (13) months following the date hereof, if there is any balance remaining in your WellDeserved account that is attributable to the store credit Award provided to you in this email, this balance shall be subject to an account maintenance fee and shall be reduced by $5.00 per month (or whatever the remaining balance is if less than $5.00), on the first (1st) such month, and the first (1st) of each month thereafter, until such balance reaches zero (herein, “Balance Fee”). For example, if this email was dated January 15th, 2016, then the Balance Fee would be deducted beginning on February 1st, 2017, and on the first (1st) of each month thereafter, from any balance remaining on your WellDeserved account that is attributable to the store credit Award provided to you in this email.
